How we support you
How AFC Works
AFC helps eligible adults receive daily care from a qualified caregiver at home. MCHS supports both sides of the relationship.
Who Can Be a Caregiver
Family members (excluding spouses) and other qualified adults can serve as paid AFC caregivers, with training and oversight.
Benefits of AFC Support
Stipend support for caregivers, structured training, documentation help, and a team that understands the AFC process.
RN Oversight
A Registered Nurse helps with assessments, care plans, and ongoing health-related oversight.
Monthly Monitoring
Routine monthly visits to keep care plans current and caregivers supported.
Enrollment Assistance
We help members and caregivers navigate enrollment, paperwork, and Medicaid coordination.
